We collect only the personal information that we need for the purposes of providing quality care to our patients, including personal information needed to:

  • effectively evaluate the nature of the person’s condition and health status
  • determine appropriate treatment options
  • communicate with other providers or third-parties as deemed necessary

We make every reasonable effort to ensure that patient information is accurate and complete. We rely on our clients to notify us if there is a change to their personal information that may affect their relationship with Oakridge Chiropractic Centre. If you are aware of an error in our information about you, please let us know and we will correct it on request wherever possible.
